Insurance

All teams that participate in the 2019 Oregon HS State League will receive insurance coverage via the USAU. That insurance covers:

• All activities between January 20th and June 19th
• Practices and Ultimate-related training
• USAU sanctioned Tournaments

OBTAINING A CERTIFICATE OF INSURANCE (COI)

If you require a Certificate of Insurance (COI) for any purpose (ie, securing fields, etc), please fill out the following form and USAU will send you one:
